What to bring.
- A photo ID and your insurance card if you carry dental coverage.
- A list of current medications, including supplements and dosages. We coordinate with your physician when needed.
- Previous dental records or X-rays, if available. If not, we will capture fresh imaging here.
- Completed registration forms. See the Patient Registration page for the easy way to do this before you arrive.
- A few minutes early. Arrive 10 to 15 minutes before your appointment so we can settle you in without rush.
