Paperwork, handled before you arrive.
We will email you the new-patient forms when you schedule. Complete them on your couch, your boat, or your phone — and walk in ready to go.
When you schedule your first appointment, we email you the registration packet directly. It takes most patients 8–10 minutes to complete. You can fill it out on a phone, tablet, or computer — whichever is easiest. The completed forms come straight back to us, securely, before your visit.
If you prefer paper, no problem. Let us know when you book and we will have a printed packet waiting at the front desk when you arrive. Just plan to come about 20 minutes early so you can take your time with it.
What is in the packet.
- Patient information form. Your basic contact details, preferred name, and how you would like to be reached.
- Medical history. Current conditions, medications, allergies, and any recent surgeries.
- Dental history. When you last saw a dentist, any prior work, and any chronic concerns we should know about.
- Insurance information. Carrier, member ID, and group number if you have dental coverage.
- Consent forms. Standard HIPAA notice and treatment consent — the same forms every dental office uses, explained in plain language.

Our online registration portal is rolling out in phases. Until it is fully live, every new patient gets their forms by email — no portal account required.
